-Name
-Parents Name (owner / your names)
-Travel Destination
-Emergency Phone Numbers
-Health or Special Instructions
WRITE IN THE NOTES BOX THE BASIC INFORMATION YOU WOULD WANT ON YOUR TAG. DOUBLE AND TRIPLE CHECK INFORMATION IS CORRECT BEFORE PLACING ORDER AS I WILL NOT DOUBLE CHECK SPELLING OR NUMBER CORRECT.
